All student-athletes please be advised that the necessary forms and paperwork to participate in Mansfield Athletics are now available electronically. You can follow the following link which has directions on how to proceed with the electronic versions of the forms:

All student-athletes are required to complete a physical with their primary care physician yearly. You need to print out the pre-participation form from the list of forms that are on the SportsWare site and take that form with you to your physical for your physician to complete. Once completed, that pre-participation physical form should be uploaded to the SportsWare site by hitting the "add" button on the forms page, once you have logged in. You should also keep a hard copy of your physical to bring back to campus with you once you return to campus.

Effective August 1st, 2022 the NCAA has added a requirement that all NEW student athletes (including those who "try-out") must provide results of their sickle cell status prior to participation.
NCAA Bylaw Update:Â
17.1.5.1 Sickle Cell Solubility Test. The examination or evaluation of student-athletes who are beginning their initial season of eligibility and students who are trying out for a team shall include a sickle cell solubility test (SST), unless documented results of a prior test are provided to the institution.
Therefore, for new and incoming student athletes to meet this requirement, you must:

These new requirements apply only student-athletes new to Mansfield (both transfers and first year students) for the 2025-2026 academic year. Student-athletes who have previously been cleared do not need to take any action as they have previously provided proof of sickle cell status that meets NCAA requirements. Â

Note that when you are filling out the insurance tab, there is a button to upload the front and back of your insurance card to the SportsWare system. Once you have uploaded your current insurance card, open the document to be certain it is legible.

All forms are to be completed electronically and uploaded on the SportsWare website. NO PAPER FORMS WILL BE COLLECTED
All forms need to be completed in their entirety by August 1st, 2025. Failing to do so will delay your ability to begin participation with your team upon your arrival to campus.
